WebMay 24, 2024 · The PCC believes that primary care is the foundation of a successful ACO that uses transparent performance measures to achieve the Quadruple Aim of improving population health, improving patients' experience of care, reducing the total cost of care, and improving the work life of health care providers. WebThe PCC provides support to the member as they navigate the health care system. The PCC may work closely with member's case management team to resolve access, medical and psychosocial related issues. This position must be able to efficiently manage and prioritize an assigned caseload of members and/or families.
